The following are Printable Forms that must be signed and returned by US Mail as original documents. If you are in the area and want to drop the forms off in person, you can drop them to the Registration Desk at the Main Office.

US Resident Background Check

  • Complete all sections of form, do not leave any sections blank or unanswered.  Required references cannot be family or relatives.
  • Make sure each applicant signs the document.
  • Enclose a color copy of driver’s license for each applicant
  • Enclose a check in the amount of $25.00 per applicant, made out to Sundance 1 RV Resort.
  • Mail to Sundance 1 RV Resort, 1703 N Thornton Road, Casa Grande, AZ 85122.  Attention: Park Manager

Canadian Background Check

  • Complete all sections of form, do not leave any sections blank or unanswered.  Required references cannot be family or relatives.
  • Make sure each applicant signs the document.
  • Enclose a color copy of driver’s license for each applicant
  • Also include a color copy of Canadian Passport for each applicant
  • Enclose a check in the amount of $50.00 USD per applicant, made out to Sundance 1 RV Resort.
  • Mail to Sundance 1 RV Resort, 1703 N Thornton Road, Casa Grande, AZ 85122.  Attention: Park Manager
